We will need the following detail:

1.  The name of the person requiring assistance:

2.  The name and details of the person organising the assistance if not the patient/attendee

3.  The address for collection

4.  The journey details:- date/appointment place and time

5.  Their mobility condition - requires walking stick/frame, wheelchair for example.

Please Note:  There is no fixed charge for assistance although we do accept voluntary anonymous donations through our envelope scheme
